• Fluency in English and Spanish
• Very strong customer service skills
• Computer proficiency and ability to work under pressure
• Bachelors Degree and two (2) years related work experience in health care

As a Care Manager, your position will include but is not limited to:

• Engaging with patients, coordinating and developing care plans with the clinical team and the patient’s input.
• Ensuring that new clients are evaluated for, and receive, all appropriate entitlements, connecting the patient with outside agencies as needed.
• Meeting with patient regularly to assess their understanding of medication regime, medication compliance and side effects.
• Working with the clinical team to ensure patient’s understanding of, and compliance with care plan.
• Provide outreach/enrollment services for patients requiring assistance with obtaining insurance coverage.
