We are Committed to Patient-Centered Care
Thank you for choosing Settlement Health as your trusted healthcare provider. Here at Settlement Health, we want to build a trusting relationship with you and your loved ones. We want to learn about you, your traditions, your goals, and your family. We understand that no one knows you better than you do, so we want to work together to help you achieve your health goals.
Your Team
To help you meet your healthcare goals, we have a team dedicated to work with you. At each visit, you will check-in with our staff. They will make sure we have accurate information needed for your visit. Then, you will meet your Medical Assistant who will start the conversation before you are visited by your Physician, Physician Assistant, or Nurse Practitioner. Your provider will partner with you to address any issues that might come up, and if the need arises, they will connect you with your other team members. These other members may include a Care Manager who can provide education about diseases and medications, and connect you to community resources. You may also be connected with other services such as nutrition counseling, foot specialists, social work, and support and wellness groups. While we do offer many different types of medical practices here in our clinics, including adult medicine, family medicine, women’s health, prenatal care, pediatrics, and behavioral health, there may be times when you need to see a specialty not offered at Settlement Health. In these cases, we have Referral Coordinators who work with you to get your appointment scheduled. We believe that working together as a team, we can do a lot more than any individual could.
Your “Patient-Centered Medical Home”
Because of this team-based approach, we call ourselves a “Patient-Centered Medical Home”. This means that we want you to think of us first, as your “home”, anytime you have a medical concern. It also means that we will reach out to you about your health from time to time. We want you to work with your team, share your concerns, questions, thoughts, and opinions, and let us know if you do not understand something we are saying.
Your Role on the Team
As a valued team-member, we pledge to respect you, and we ask for your respect in return. We know that life gets busy and you may not be able to make every appointment.
